Safe Environment Checklist
Catholic Diocese of Peoria
The Catholic Diocese of Peoria requires supervisors of all employees/volunteers to ensure that the following requirements are completed or are in process prior to the commencement of work or volunteer duties. The policies/forms described below can be downloaded at www.cdop.org.
1. Ensure all employees/volunteers have received the appropriate policies: • Policies and Procedures Relating to Allegations of Sexual Abuse of Minors • Code of Pastoral Conduct
• Harassment Policy
• Bullying Policy
• Communication Policy
• Employee Code of Conduct (employees only)
• Volunteer Code of Conduct (volunteers only)
2. Ensure that each new employee/volunteer creates a CMG Connect account. • Log into https://Peoria.cmgconnect.org
• Create a new user account
• Complete the required trainings and background checks
3. All school employees are required to complete:
• Safe Environment Training for School Employees (CMG Connect)
• Driving Requirements – Peoria (CMG Connect)
• Bloodborne Pathogens (CMG Connect or similar training)
• Sexual Harassment & the Church (CMG Connect)
• Digital Fingerprints through Accurate Biometrics or Flawless Biometrics –
must have both FBI and IL State Police results
• CANTS form – complete and submit to SafeEnvironment@cdop.org •
Mandated Reporter Training (https://mr.dcfstraining.org)
• DCFS Acknowledgment of Mandated Reporter Status Form
4. All school volunteers are required to complete:
• Safe Environment Training – Peoria (CMG Connect)
• Driving Requirements – Peoria – (CMG Connect—required for volunteer drivers) •
Electronic Criminal Background check – Selection.com (embedded in CMG Connect safe environment training)
• CANTS form – complete and submit to SafeEnvironment@cdop.org
