(Divisions may be combined due to the number of entries.)

Cost: $48.00/Team (Make checks payable to ICS 3 on 3 Classic)

SAVE YOURSELF SOME TIME!! Print out and fill in Waiver Form, ICS 3 on 3 T-Shirt Form, Rule & Entry Form.

• A waiver must be signed by an adult for each participant.

• Game times will be posted on our website after 12:00 p.m. on Friday, March 6^th.

• Each team member must check in at least ½ hour prior to their first game.

ICS 3 on 3 T-Shirts can also be ordered in advance and picked up on the day of the

tournament by calling Melco Products/MC Sports at 309-734-5912.
